Questions

Does this include state tax?
No. It covers federal income tax and FICA only. State and local tax vary enormously — from nothing at all in several states to over 10% — so a figure here will be too high if your state taxes income.
Why does a 401(k) contribution not reduce Social Security tax?
Traditional 401(k) contributions come out before income tax but after FICA. So they lower your income tax bill while Social Security and Medicare are still charged on the full amount.
